Family Sign-Up

Sign your family up for 4C Sensory Center giving programs.

4C supports children and teens with autism, ADHD, sensory needs, and other developmental differences, and their siblings too. Fill out one form for your whole household and choose every program you would like to be considered for.

📋 What you will need

  • Proof of your child's diagnosis, such as a letter or report from a doctor, therapist, or evaluation.
  • A recent photo of each child.
  • Clothing, shoe, and coat sizes for each child in the home.

After you submit this form, you will email your proof of diagnosis and photos to us. We will show you how on the next screen.
